Introduction: Ocular trauma is an important cause of unilateral visual impairment and blindness. Among several agents ocular trauma, blunt the most common can lead to secondary glaucoma. Secondary glaucoma due be unnoticed undetected until formation glaucomatous optic neuropathy (GON) occurs. This case might not neglected. Delay in treatment progression GON. Case Presentation: A 68-year-old woman presented outpatient clinic Undaan Eye Hospital complaining decreased vision pain left eye after eye. Her was limited detecting hand motions (1/300) intraocular pressure (IOP) 37.8 mmHg. On slit lamp biomicroscopic examination, pupil mid-dilated, lens opacification phacodenesis were detected. The cup-to-disc ratio (CDR) increased (0.8-0.9) through funduscopic examination. subluxated diagnosed for patient given topical oral antiglaucoma medications evaluated one week drug use. Since IOP remained elevated medical therapy, trabeculectomy conducted. Bleb formed postoperatively getting normal without medications. Conclusion: Blunt Thorough examinations immediate treatments should conducted preserve patient’s prevent further nerve damage.
